HIPAA Consent

HIPAA provides safeguards to protect your privacy regarding Protected Health Information (PHI). Our policies include: (1) Keeping patient information confidential except as necessary to provide services. (2) Reminding patients of appointments via phone, email, or mail.

(3) Vendors may access PHI but must abide by HIPAA confidentiality rules. (4) You agree to government/insurance inspections in the normal performance of their duties. (5) Your confidential information will NOT be used for marketing of products or services.

Financial Consent

You understand that a credit card may be kept on file and any remaining balances for services rendered shall be paid in full. All programs are auto-renewing. There are no refunds or exchanges. You certify that you are an authorized user of the credit card.

Shipping Authorization

All prescription medications are dispensed according to state and federal law. You disclaim and hold harmless CareValidate and trimmedi for any delays or errors during shipping. Medication is considered dispensed when it is signed out for shipping, not when it arrives.

The health assessment available on this website does not create a provider-patient relationship between the individual completing the assessment and Trinity HealthCare Supply, LLC d/b/a trimmedi. Medical services, including clinical evaluation and prescribing, are provided by Usable Health, PC and its affiliated professional medical practices. Licensed providers within the Usable Health, PC network apply clinical protocols and exclusionary criteria to determine whether an individual qualifies for treatment. A provider will conduct a telehealth consultation to evaluate medical appropriateness before any prescription is issued. Providers retain sole discretion over all prescribing decisions. A prescription is never guaranteed.

Pharmacy Partners: Prescriptions are filled by independently owned and operated state-licensed 503A compounding pharmacies within the trimmedi pharmacy network. Each pharmacy maintains its own state board of pharmacy licenses and operates under applicable federal and state compounding regulations.
