We collect several types of information including: personally identifiable information (name, address, email, phone, account info); health-related information such as clinical history; information about your Internet connection and device; usage details; and any other information we specifically request.

We collect this information: directly from you when you provide it to us; automatically as you navigate through the Platform (usage details, IP address, cookies, web beacons, and other tracking technologies); and from third parties such as physicians, medical professionals, and pharmacies.

The Platform may utilize cookies to improve functionality. Cookies tell us which parts of the Platform you've visited. We use cookies to identify and track visitors and their usage. Visitors who do not wish to have cookies placed on their computers should set their browsers to refuse cookies before using the Platform.

We use Intuit's Quickbooks and/or Stripe as our payment processor. A cookie is stored on your browser to assist in detecting and preventing fraud. These are session cookies that typically only remain on your browser for 24 hours.

III. How We Use Your Information

We may use your information to: complete registration or transactions; provide treatment, payment, or healthcare operational purposes; communicate about healthcare services; administer your account; operate the Platform; provide technical support; create de-identified aggregate statistics; measure effectiveness of communications; and comply with applicable laws.

If you provide your mobile phone number and opt in, you consent to receive recurring text messages (SMS and MMS) from us. Message and data rates may apply. Reply STOP to unsubscribe at any time. Consent to receive text messages is not a condition of any purchase or use of our services.

IV. Disclosure of Your Information

We may disclose your Personal Information to: service providers that assist in maintaining and improving the Platform; medical providers including physicians, pharmacies, and laboratories; our affiliates and their employees; third-party credit card processors; and entities assisting with marketing and advertising.

No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. All categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.

V. Your Options

You can set your browser to refuse all or some browser cookies, or to alert you when cookies are being sent. If you disable or refuse cookies, some parts of our Platform may be inaccessible or not function properly. We do not currently respond to 'do not track' signals.

VI. Data Security

We have implemented measures designed to reasonably secure your Personal Information from accidental loss and from unauthorized access, use, alteration, and disclosure. Unfortunately, the transmission of information via the Internet is not completely secure. Any transmission is at your own risk.

CareValidate is not responsible for what third parties do in connection with their websites or how they handle your Personal Information. VIII. No Services to Persons Under 18

CareValidate does not knowingly provide services to minors under 18.

IX. California Resident Privacy Rights

If you are a resident of California, the California Consumer Privacy Act (CCPA) provides you with additional rights regarding your Personal Information. Your rights include: Right to Know, Right to Deletion, Right to Opt-out of the Sale, and Right to Non-Discrimination.

To opt out of the sale of personal information, you may click the 'CCPA Do Not Sell My Personal Information' link in the footer of our website and modify your cookie settings.

The health assessment available on this website does not create a provider-patient relationship between the individual completing the assessment and Trinity HealthCare Supply, LLC d/b/a trimmedi. Medical services, including clinical evaluation and prescribing, are provided by Usable Health, PC and its affiliated professional medical practices. Licensed providers within the Usable Health, PC network apply clinical protocols and exclusionary criteria to determine whether an individual qualifies for treatment. A provider will conduct a telehealth consultation to evaluate medical appropriateness before any prescription is issued. Providers retain sole discretion over all prescribing decisions. A prescription is never guaranteed.

All claims and results referenced on this website reflect self-reported data from members enrolled in the trimmedi healthcare membership program, which includes telehealth consultations with licensed providers and, where clinically appropriate, compounded GLP-1 medications. Members reported their weight at initial intake and at regular intervals thereafter. Individual results vary and depend on factors including starting weight, program adherence, diet, exercise, and the clinical recommendations of the treating provider. Compounded medications are prepared by state-licensed 503A compounding pharmacies. These medications are not FDA-approved and have not been evaluated by the FDA for safety, efficacy, or quality. The decision to prescribe compounded medications is an individualized clinical determination made by the treating provider based on the patient's medical history, current health status, and telehealth evaluation. Trinity HealthCare Supply, LLC d/b/a trimmedi does not manufacture or compound medications. Actual medication appearance may differ from website imagery.

Pharmacy Partners: Prescriptions are filled by independently owned and operated state-licensed 503A compounding pharmacies within the trimmedi pharmacy network. Each pharmacy maintains its own state board of pharmacy licenses and operates under applicable federal and state compounding regulations.

Individual results vary based on starting weight, medical history, program adherence, diet, and exercise. The weight loss figures referenced on this website are based on self-reported data from trimmedi members and are not derived from clinical trials or peer-reviewed studies. Not all members experience the same results. A prescription is never guaranteed and is at the sole discretion of the treating provider. Consult your healthcare provider before beginning any weight management program.
